Discord staff application template: questions you can copy and paste

Five ready question sets for staff, moderator, helper, event-team and clan applications on a Discord server. Every question is 45 characters or shorter, because that is the limit Discord puts on a question label in a pop-up form, so you can paste them into any form bot (or a Google Form) without trimming.

Before you write questions: what a good staff application finds out

The templates

Short questions use a one-line answer box; the rest use a paragraph box. Each set is also built into Formwell as a template: open "As Formwell commands" for the one command that creates the form with all five questions (pick the set from the template list), or the values for each /form add-question if you would rather add them one at a time. To change a set (add, drop or reorder questions) and copy the result, open it in the application form builder.

Formwell: these forms inside Discord, free

Members click Apply and answer in a Discord pop-up. Answers land in your review channel with Accept and Deny; Accept gives the role and DMs the member. Free: 3 forms, 5 questions each, unlimited submissions. Set up with 3 slash commands.

Add Formwell to your server

Need more? Pro: up to 50 forms, multi-page forms (25 questions), cooldowns, CSV export, $5/month per server. Run /premium in your server to upgrade.

Formwell is new: it runs in our own test server and we are looking for its first real servers. Bugs: formwell@tuper.app.

How to set this up in Discord (with Formwell)

  1. Add the bot with the button above. It asks for Manage Roles (to give the role on accept) and permission to post in channels.
  2. /form create name:staff review_channel:#staff-apps accept_role:@Trial Mod creates the form. The review channel should be private to your staff.
  3. Add the five questions: add template: to the create command and pick a set from the list (staff, moderator, helper, event team, clan), or add them one by one with /form add-question (values under "As Formwell commands"). Edit any question later with /form edit-question.
  4. /form publish form:staff channel:#apply posts the panel with an Apply button.
  5. When someone applies, their answers appear in #staff-apps with Accept and Deny. Accept gives @Trial Mod and DMs them; Deny asks you for a reason and DMs it.

Put Formwell's role above the role it gives (Server Settings > Roles), or Discord will not let it assign the role.

Google Form or a Discord form?

A Google Form has no length limit on questions and any number of pages, which suits a 30-question application. The cost: applicants leave Discord, you have to match the Google answers to a Discord user by hand (people mistype their username), and nobody gets a role or a reply automatically. A form inside Discord knows exactly who applied, and the decision gives the role and sends the reply in one click. A common setup is a short Discord form as the first filter, then a voice or text interview for the people you like.

Longer applications

Discord shows at most 5 questions per pop-up. If you really need more, either split the form into steps (Formwell Pro does this, up to 25 questions, $5/month per server) or ask the extra questions in the interview. These work well there:
