Discord whitelist application bot for Minecraft and FiveM servers
A whitelist application on Discord usually works like this: the player answers a few questions, staff read them, and an accepted player gets a Whitelisted role. The game server then lets in players who have that role. This page gives you the questions, the Discord side with a free bot, and the honest part: how the role reaches the game.
Step 1: the questions (copy and paste)
Every question fits Discord's 45-character limit for form labels. Want to add a Java/Bedrock or age question first? Open the set in the application form builder, edit it and copy the commands.
Minecraft SMP whitelist
1. Minecraft username (Java or Bedrock?)
2. How old are you?
3. How did you find our server?
4. What would you build here first?
5. Have you read the rules? What is rule 3?
As Formwell commands
/form create name:whitelist review_channel:#wl-review template:Minecraft SMP whitelist
Or add the questions one at a time:
/form add-question form:whitelist label:Minecraft username (Java or Bedrock?) style:Short (one line)
/form add-question form:whitelist label:How old are you? style:Short (one line)
/form add-question form:whitelist label:How did you find our server? style:Short (one line)
/form add-question form:whitelist label:What would you build here first? style:Paragraph
/form add-question form:whitelist label:Have you read the rules? What is rule 3? style:Short (one line)FiveM / GTA RP whitelist
1. Character name and age
2. Your character's backstory
3. Explain RDM and VDM in your own words
4. What is metagaming? Give an example
5. You're held at gunpoint. What do you do?
As Formwell commands
/form create name:whitelist review_channel:#wl-review template:FiveM / GTA RP whitelist
Or add the questions one at a time:
/form add-question form:whitelist label:Character name and age style:Short (one line)
/form add-question form:whitelist label:Your character's backstory style:Paragraph
/form add-question form:whitelist label:Explain RDM and VDM in your own words style:Paragraph
/form add-question form:whitelist label:What is metagaming? Give an example style:Paragraph
/form add-question form:whitelist label:You're held at gunpoint. What do you do? style:ParagraphFor roleplay servers, the rule-knowledge questions (RDM, VDM, metagaming) do the filtering: people who copy answers from another server give themselves away. Ask for the backstory in a paragraph box and read it; it shows effort and writing level at once.
Step 2: the Discord side (free, about 3 minutes)
- Create a role, e.g.
@Whitelisted, and add Formwell with the button below. /form create name:whitelist review_channel:#wl-review accept_role:@Whitelisted- Add the five questions: put
template:on the create command and pick "Minecraft SMP whitelist" or "FiveM / GTA RP whitelist", or add them one by one with/form add-question(values under "As Formwell commands" above). /form publish form:whitelist channel:#whitelist
Players click Apply, answer in a Discord pop-up, and your staff click Accept or Deny in #wl-review. Accept gives @Whitelisted and DMs the player; Deny DMs your reason, so nobody is left asking "did I get in?".
Members click Apply and answer in a Discord pop-up. Answers land in your review channel with Accept and Deny; Accept gives the role and DMs the member. Free: 3 forms, 5 questions each, unlimited submissions. Set up with 3 slash commands.
Add Formwell to your serverNeed more? Pro: up to 50 forms, multi-page forms (25 questions), cooldowns, CSV export, $5/month per server. Run /premium in your server to upgrade.
Formwell is new: it runs in our own test server and we are looking for its first real servers. Bugs: formwell@tuper.app.
Step 3: from the Discord role to the game
Formwell does not talk to your game server. It gives the Discord role. The game side is done by a plugin or resource on your server that checks that role, and these exist for free:
- Minecraft (Paper/Spigot): DiscordSRV links Minecraft accounts to Discord. In its
linking.yml,Require linked account to playplusSubscriber role: Require subscriber role to join: truewith your@Whitelistedrole ID lets in only linked players who have the role (the option is named after Twitch subs, but any role works). Players link once with the code DiscordSRV gives them. - FiveM: Badger_Discord_API reads a connecting player's Discord roles (
GetDiscordRoles); a small whitelist script or a framework's Discord whitelist option denies the connection when@Whitelistedis missing. Players need Discord open on the same PC so FiveM knows their Discord ID. - No plugin: keep a manual whitelist and add the Minecraft name from the accepted application yourself (
/whitelist add name). Fine for a small SMP.
Step-by-step Minecraft setup (DiscordSRV linking.yml, Bedrock players, current whitelist plugins, fixes): Minecraft whitelist with a Discord bot. FiveM: FiveM whitelist guide.
Paid whitelist bots built only for FiveM/RedM (for example Whitelist Pro, 8,810 servers in Discord's App Directory on 2026-11-01, prepaid after a 3-day trial, or WhitelistM, 1,870 servers, free with a paid tier) write the approval straight into the server database (vRP, QBCore, ESX). If you want that and play FiveM, they do more than Formwell. Formwell is the simpler, free choice when a Discord role is enough, and it works for any game.
Good whitelist habits
- Use a private review channel and a reviewer role, so applicants never see each other's answers.
- Write the deny reason as if the player will read it, because they will.
- Re-publish the panel when you change questions; old answers stay in the review channel.
