Discord application form builder (free)
Build a staff, whitelist or recruitment application for your Discord server here, then copy it out: as the exact Formwell slash commands to paste into Discord, as plain text for a pinned message or a Google Form, or as a link to share the draft with your other admins. Start from one of 10 ready question sets or write your own. Every question label is checked against Discord's 45-character limit for pop-up forms.
Nothing you type is sent to us or stored: the form lives in this page and in the link's # part, which browsers never send to a server.

1. Formwell commands (paste into Discord)
Paste one line at a time into any channel of your server. With an unchanged ready set the first line alone creates the whole form. Replace #applications and #apply with your channels if Discord does not pick them up.

Anyone who opens the link sees this form in the builder and can change their own copy.

Each set is built into Formwell. Pick it from the template list of /form create; click a name to open it in the builder above. The questions themselves are on the staff application template and whitelist application pages.

Question styles
Short and Paragraph are text boxes and are live. Dropdown (pick one from a list), Multiple choice (pick one), Checkboxes (pick any), File upload (screenshots, proof) are built and pass our automated tests, but nobody has tried them in a real server yet, so the builder marks them live test pending; the ready sets use text questions only until that test. Choices go after options: separated by | (dropdown up to 25, the others up to 10). The free plan allows 5 questions per form (one Discord pop-up page); Pro allows 25 across pages.
