Minecraft UUID lookup: names to UUIDs and whitelist.json (free)
Paste up to 20 Minecraft Java usernames, one per line or separated by commas, and get each player's UUID with and without dashes, a ready whitelist.json to drop into your server folder, and the matching whitelist add console lines. Names that do not exist are flagged, so a typo in an application does not end up on your whitelist.
Results
whitelist.json
Replace the file in your server folder (stop the server first, or run whitelist reload after saving), or merge the entries into your existing list.
Console commands
UUIDs only
Formwell runs the whole application inside Discord: members click Apply, answer in a pop-up (Minecraft name, age, why they want to join), and your staff press Accept or Deny in a review channel; Accept gives the Whitelisted role and DMs the player. Free: 3 forms, 5 questions each, unlimited submissions. Formwell can also look up the Minecraft name on the form itself, showing the UUID next to the answer; that option is built and still in its live test.

What a Minecraft UUID is, and why whitelists use it
Every Java Edition account has a fixed UUID (a 32-digit hex ID, usually written with dashes as 8-4-4-4-12). A player can change their name every 30 days; the UUID never changes. That is why whitelist.json, ops.json and banned-players.json store the UUID next to the name: rename and you stay whitelisted, and a stranger who later takes your old name does not get in.
The UUIDs here come from Mojang's public profile lookup (the same one the server uses for whitelist add), with names in their exact capitalisation.
Offline-mode servers
A server with online-mode=false does not ask Mojang. It makes the UUID from the name itself (a version 3 UUID of OfflinePlayer:<name>), so the same player has a different UUID there, and capitalisation matters. Tick the offline-mode box to get those UUIDs and a matching whitelist.json. Behind a proxy (BungeeCord, Velocity) with IP forwarding on, the backend servers use the online UUID.
Bedrock players
This tool covers Java Edition only: Bedrock gamertags are Xbox accounts and are not in Mojang's profile lookup. On a Geyser + Floodgate server, Bedrock players get a Floodgate UUID derived from their Xbox ID; add them with Floodgate's own command (fwhitelist add <gamertag>) rather than through this list.
How this differs from NameMC and other lookups
NameMC, mcuuid.net and the host-run lookup pages answer one name at a time and add profile pages, skins and name history. This page is for a whitelist admin with a batch of accepted applicants: up to 20 names in one go, typos flagged, and the output already in whitelist.json and console form, for online or offline mode. It does not show skins or name history.
Privacy and limits
The names you type go to our lookup service (formwell-api.tuper.app) and from there to Mojang; we do not store or log them, and results are cached for 10 minutes. The lookup has one shared limit for all visitors (a few dozen names a minute) so it stays inside Mojang's rate limit; if you hit it, wait a minute.
