Formwell

Discord ban appeal form template (free)

Running a server? Copy ready appeal form questions and three replies (accepted, denied, need more information). Were you banned? Build a clear, honest appeal message. Nothing is uploaded: it all runs in your browser.

Appeal form questions

Stop collecting appeals in DMs.

Formwell runs ban appeals inside Discord: the member clicks Appeal, answers in a pop-up, and your staff press Accept or Deny in a review channel; Accept can give a role back and DMs the member. Free: 3 forms, 5 questions each, unlimited submissions.

Where members can appeal

A banned member cannot see your server, so the appeal needs a place outside it: a small appeals server, a link to a form, or a bot that takes the appeal in DMs. Say where in your ban message and in your rules, so nobody has to guess.

What a fair appeal process looks like

If you were banned

Be honest, brief and specific. Say what you did, which rule it broke and what you will change. Do not argue, blame the moderator or send the appeal more than once. Alt accounts used to get back in usually end the appeal.
