Discord support ticket form: questions to copy, and when a form beats a ticket bot

A support ticket on Discord usually starts as "help pls" in a channel. The fix is a short form that asks for what a helper needs before anyone opens a thread. This page gives a five-question support form you can copy, a plain comparison with ticket bots, and the steps to run it. Form questions are 45 characters or shorter, the limit Discord puts on a pop-up form label.

The support form (5 questions)

These get a helper from "something is broken" to "I can start" without a second round of questions:

1. What do you need help with? (one line)
2. Your Discord username and in-game name?
3. What did you expect, and what happened?
4. Steps to reproduce, or a link to the message?
5. Anything you already tried?

Using a different set? Open the application form builder, paste your own questions and copy the Formwell commands.

Why these five

QuestionWhat it saves
What do you need help with? One line a helper can sort by; stops "hi, anyone here?"
Username and in-game nameDiscord shows the user, but a game, server or shop account often has another name
Expected vs happenedSeparates a bug from a misunderstanding in the first read
Steps or a message linkLets a second helper repeat the problem
Already triedSkips the "have you restarted it" round

Form or ticket bot?

Formwell is a form bot, not a ticket system. It collects structured answers and posts them to a review channel; it does not open a private thread per request, keep transcripts or number tickets. Pick by what you need:

You needBetter fit
A private back-and-forth with each member, transcripts, ticket numbersA ticket bot
Structured intake so helpers can triage before they replyA form inside Discord
Staying out of ticket threads for simple requests (role requests, refund asks, sponsor enquiries)A form with a decision: Accept or Deny, with a reason sent to the member
BothUse the form as the front door and open a thread yourself for the ones that need talking

For other form shapes see the comparison of Discord forms bots.

Run the support form in Discord

  1. Add Formwell with the button above.
  2. /form create name:support review_channel:#support-requests creates the form. Leave out accept_role if Accept should not hand out a role. Add the questions above with /form add-question.
  3. /form publish form:support channel:#get-help posts a panel with a button members click to open the pop-up.
  4. Answers appear in #support-requests with Accept and Deny. Accept DMs the member; Deny asks you for a reason and sends it.

Free covers 3 forms with 5 questions each and unlimited submissions, which fits one support form. Rules for the channel itself are in the server rules template.
