FiveM whitelist application questions: 10 to copy, and how to mark the answers
A FiveM roleplay whitelist works only if the questions tell you who will follow your rules once they are in the city. This page gives five questions for the Discord pop-up form, five short scenarios, a table of the rule terms to quiz on, a way to score answers, and the steps to run it with Formwell. Form labels are 45 characters or shorter, the limit Discord puts on a pop-up field.
The form questions (5)
These fit one pop-up. Use the long answer style for the backstory:
1. Your age and Discord username?
2. Character name and short backstory?
3. What is RDM and why is it banned here?
4. Define metagaming with an example.
5. Why do you want to join this community?Scenario questions (5)
Put these in a second form, or in a short voice interview for people who passed the first:
1. Cop pulls you over and you flee. Your RP?
2. You are shot. What do you do before EMS?
3. A friend tells you a rival's hideout. Use it?
4. You crash and are stuck in a wall. Steps?

Terms to quiz on, and what a good answer says
Swap these definitions for the ones in your own rules, and send applicants to your rules page first:

| RDM (random deathmatch) | Killing a player with no roleplay reason or warning |
| VDM (vehicle deathmatch) | Using a vehicle as a weapon with no RP reason |
| Metagaming | Using information your character could not know, such as something said on Discord voice or a stream |
| Powergaming | Forcing an outcome on another player instead of letting them respond |
| Fail RP | Acting in a way your character or the situation would not allow |
| Combat logging | Leaving the server in the middle of a scene to avoid its result |
| New life rule | After a character dies they forget how it happened and do not seek revenge |

Marking the answers

Always send a reason on Deny. Applicants who learn what to fix often return as better players.
